For anyone over 18 and living in our region, the GetaKit program offers free at home HIV self-test kits shipped directly to your home. Requisitions and swabs for other sexually transmitted infections such as chlamydia, gonorrhea, and syphilis are also available. This service is free and no health card is required. Smiths Falls Community Health Centre
Opening in early 2010, the Smiths Falls Community Health Centre provides health services to people who live in and around Smiths Falls.  Health-care providers include physicians, nurse practitioners, nurses, social workers, dietitians and health promotion staff. Like all the other 74 Community Health Centres in Ontario, Smiths Falls Community Health Centre is community-governed and community-driven.
 Doctors and Health Care Providers
If you need help finding a family doctor or nurse practitioner, Health Care Connect will assist you with your search. Their site includes a form you can complete listing basic information in order to start the process of finding a family health care provider.

You can also call the program at 1-800-445-1822, Monday to Friday, from 9 a.m. to 5 p.m. to begin the same process.

Community Care Access Centres (CCAC)
South East Community Care Access Centre Community Care Access Centres (CCAC) is funded and legislated by the Ontario Ministry of Health and Long-Term Care. Your CCAC connects you with the care you need, at home and in your community:
  • Stay in your own home longer by providing Care in Your Home and by coordinating Care in Your Community, including specialized support services;
  • Information about Long-Term Care Options if it becomes too difficult for you to live independently at home.

CCACs are staffed by caring and knowledgeable professionals who will assess your needs, determine your requirements for care, answer your questions and develop a customized care plan that meets your individual needs. Then, if services are provided to you by your CCAC, we’ll arrange for quality health-care professionals – nurses, physiotherapists, social workers, registered dietitians, occupational therapists, speech therapist and personal support workers – to provide care and supportive services to help support you at home and help you enjoy the best possible quality of life.

Note that there is generally a fee associated with community services. Subsidies are available based on eligibility requirements. Your CCAC Case Manager can explain what services are covered by the CCAC and which have a fee, and how to apply for subsidies.

Community and Primary Health Care (CPHC) exist to provide universally accessible and comprehensive primary health care and community support services in the promotion and building of a healthier community.

With 11 sites across Lanark, Leeds and Grenville, we offer a wide range of community and health services for our diverse population.

CPHC Seniors Exercise Program is open to those 50 years plus, Starting Sept. 12,  the fitness classes will resume again  The classes will be held twice a week on Mondays and Fridays, 10 – 11 a.m., at the Seniors Centre beside the rink.  Contact Karen Forbes at 613-283-4138 to register for the classes. The Alzheimer Society of Lanark County offers an ongoing support group for individuals with Alzheimer’s disease or related dementia and their care partners. Among other programs and services offered by the agency are an Adult Day Service and a Day Away Respite Service.

Lanark County Mental Health 

Lanark County Mental Health is a community based service responding to adults and transitional aged youth (17 years and over) who experience mental health concerns such as suicidal thoughts, trauma, anxiety, depression or symptoms of psychosis. Counselling and other clinical services are also available to support adult victims of domestic violence and/or sexual assault.
